Magnesium L-threonate has the same structural problem Black Seed Oil has with thymoquinone and Tongkat Ali has with eurycomanone: the trial-grade material is a specific branded extract, and most retail bottles are something else. Slutsky et al. (2010) showed that magnesium L-threonate elevated cerebrospinal fluid magnesium in rats — the result behind the "uniquely crosses the blood-brain barrier" marketing claim. No published human pharmacokinetic study has measured CSF magnesium before and after oral magnesium L-threonate to confirm the same enhancement in humans. The entire human evidence base sits in a single 44-participant Biogerontology RCT (Liu et al., 2016): twelve weeks of ~1.5–2 g/day elemental magnesium as the L-threonate salt, with cognitive-composite improvement. That RCT tested the Magceutics / NBTY / MIT-licensed Magtein® brand, not a generic chemical-purity-equivalent bought at a lower price. The brand-affiliated supply chain is part of the intervention.
Magnesium content on a label is almost always quoted as the compound weight of the magnesium salt, not the elemental magnesium payload. The four common forms sit at very different percentages:
A "500 mg magnesium threonate" front-of-bottle with no elemental-Mg column on the Supplement Facts panel is a common retail shortcut. Comparing that label to a citrate or oxide capsule on the compound axis is an apples-to-oranges comparison without a calculator — and if only the compound number is on the bottle, that is itself a signal.

USP Verified is the United States Pharmacopeia dietary supplement verification mark — it checks label-claim accuracy, contaminants, cGMP, and dissolution. NSF International’s NSF/ANSI 173 standard covers the same family of checks. ConsumerLab.com runs independent product testing and publishes subscriber-facing reviews. None of the three verifies the Magtein® trademark authenticity in this category — the brand-side Certificate of Analysis and the MIT / Magceutics license provenance are the load-bearing evidence. A $5–$10/month generic "magnesium threonate" capsule is not the same intervention as $30–$50/month Magtein® at the Liu 2016 elemental dose. Liu’s cognitive-composite result was generated in 44 humans on the Magceinics-licensed material at ~1.5–2 g/day elemental magnesium; a generic chemical-purity-equivalent bought off Amazon at 1/5 the price, with a fraction of the elemental dose and no provenance disclosure, is a different risk-adjusted profile. Cheap is not safer. Cheap is unpredictable, and unpredictable is not the same intervention.
